SEMPSL Player Registration Form

Every new player must fill out an "Amateur Player Registration Form". Team managers should fill out the "Team Information" area and players should fill out the "Player's Information" area.

All information must be typed into the form. There are grey boxes in the form indicating where each piece of information should be entered. Simply click on the grey box and then type over it. The box will disappear. I suggest that the manager get the player information and enter it, rather than sending a copy of the form to each player. Once all the information has been typed, it should be printed and signed by the player.

It is very important that everything is filled out correctly to ensure that the player's registration is not rejected by the state. Below is a list of guidelines to help you fill this form out correctly.

PLAYER INFORMATION

LINE 1: Enter the player's last name, first name and middle initial (if one exists).

Indicate the gender of the player. I suggest typing the full word, so that it is not confused for the middle initial.

Lastly, enter the last four digits of the player registration number as listed on the player card.

LINE 2: Player's address: street address, city and zip code. Note: player's should use their local address.

LINE 3: Player's date of birth and country of birth.

LINE 4: Player's phone number, business phone number, and e-mail address (if available).

LINE 5: If the player played organized soccer in another State, indicate which state.

LINE 6: Same as line 3 if not U.S.A.

LINE 7: If the player played organized soccer in another country, indicate the league and team for which they played, and the last year that the player was with the team.

TEAM INFORMATION

LINE 8: State code (26) and State Association (Michigan Soccer Association) - this should already be on the form

LINE 9: League Number (enter 01) and League Name (enter South East Michigan Premier Soccer League).

LINE 10: Team number and Name - these numbers are the fourth and fifth numbers of the full player registration number, as seen on a player card. For example, if the registration number is 001262239, enter 26.

LINE 11: Manager's name, street address, city and zip code.

LINE 12: Manager's phone number, business phone number and e-mail address.

RELEASE AND DISCLAIMER

LINE 13: The player must sign and date the form.

LINE 14: The manager must sign and date the form.

LINE 15: LEAVE BLANK.
