Any player that is not a U.S. citizen must fill out an "International Clearance Request Form". This form is pretty simple, so the instructions are sort of stupid.

It is very important that everything is filled out neatly and correctly to ensure that the player's registration is not rejected by the state. Below is a list of guidelines to help you fill out the pass cards correctly.

  Mark the gender of the player. It is not always obvious by the name.
BIOGRAPHICAL INFORMATION
LINE 1: Players name - last name, first name, middle name.
LINE 2: Mother's name - last name (including maiden name), first name, middle name.
LINE 3: Father's name - last name, first name, middle name.
LINE 4: Permanent address.
LINE 5: Present address - only necessary if it is different than the permanent address, such as if you currently are living in a dorm.
LINE 6: Date of Birth - NOT today's date!
  Social Security Number - this is actually for the SSN and not the player ID, but the field is optional, so it may be left blank.
  Place of birth - enter the city and state.
  Country - enter the country in which the player was born.
LINE 7: Citizenship - enter the country to which the player is a citizen.
  Telephone number - enter the player's telephone number.
  Contact number in U.S. - only necessary for minors and if it is different than the telephone number entered previously.
 
REQUEST FOR INTERNATIONAL TRANSFER CERTIFICATE
LINE 8: Last Club Participated With - if the player was on a registered professional or amateur team, enter the team name, otherwise leave lines 8 through 10 blank.
  League - enter the league with which the team played.
  Country - enter the country from where the team played.
LINE 9: Date of last game - the date of the last game with the registered team.
  Date Clearance Requested - the date in which the player requested clearance from the registered team
 
SIGNATURES
LINE 10: The player must sign and date the form.
LINE 11: If the player is a minor, a parent or guardian must also sign the form, otherwise leave it blank.
