SEMPSL Player Pass Card

Every player must have a player pass card. Player's without a pass card are not allowed to play unless all registration information for that person is filled out and turned in to Dean. While waiting for the return of the pass cards, these players must present their driver's license or other similar form of identification.

It is very important that everything is filled out neatly and correctly to ensure that the player's registration is not rejected by the state. Below is a list of guidelines to help you fill out the pass cards correctly.

FRONT
LINE 1: Check Amateur.
LINE 2: ID Number - The league code (001) is the first three digits, the team number is the next two digits, and the last four digits come from the back of the player's pass card.
LINE 3: Player's name - enter the last name first then the first name.
LINE 4: Date of birth - NOT today's date!
LINE 5: The player must sign the card.
BOX Affix a color picture of the player. Any headshot picture will do. Cutting out the picture from a color photo-copy of the player's driver's license works well.
 
BACK
LINE 1: Registration number - this should already be stamped on the card.
LINE 2: Today's date - yes, really today's date this time.
LINE 3: Season - enter 2002.
LINE 4: Club - enter the team name.
LINE 5: League - enter SEMPSL.
LINE 6: Registrar - LEAVE BLANK.
LINE 7: Pass issued - LEAVE BLANK.
